Summary
An investigation describes a welder’s fall during torch removal of a satellite platform ladder and cage. Bottom braces and a securing rope failed; his fall protection was attached to the ladder being removed. He fell to a deck approximately 30 feet below, then into water, sustaining a broken leg.
