Summary
This investigation examines casing failure and drilling-mud pollution during deepwater well operations. Drill-pipe wear at a buckled liner connection caused leakage, with synthetic-base mud reaching the seabed. It describes diagnostic logging, cement plugging and leak squeezing, alongside proposed improvements to casing design, wear monitoring, change management and engineering training.
