Summary
An operator suffered severe burns when heated triethylene glycol escaped during clearance of a blocked level-safety bridle on Main Pass 270 B. The investigation found that testing procedures were followed, but thermal exposure was overlooked in hazard analysis and protective equipment was inadequate. Treatment was followed by three days of restricted work.
