Summary
Investigation of a Ram-Powell crane boom-wire failure during placement of a glycol tote. The falling boom and tank damaged deck containment, with glycol subsequently discharged through the sump system. Internal corrosion and ineffective lubrication were identified. Recommendations specify periodic replacement of high-strength wire rope and consideration of more readily lubricated or corrosion-resistant alternatives.
