Summary

Investigation of a welder’s fatality during offshore platform decommissioning. A tank skid with attached vent booms rotated and fell into the sea during weld cutting, carrying the worker overboard. The panel examines ambiguous cutting instructions, omitted preparation planning, generic job safety analyses, inadequate oversight and failure to stop after unexpected movement.
