Summary
This investigation examines a contract welder’s fracture during torch cutting of pipe on a platform cellar deck. Cutting a supporting U-bolt allowed unsecured pipe to fall and trap his arm against a handrail. Findings address inadequate pipe support, hazardous body positioning and failure to communicate that the securing rope had not been attached.
