Summary
Investigation of a contractor injury during overflow-pipe removal on Pompano platform. Improper spooling on an air tugger allowed the load to drop onto a worker’s knee, causing a compound fracture. Findings address changed lift sequencing, absent tag lines, supervision and missed stop-work opportunities; corrective actions revise JSEA reviews and supervisory arrangements.
