Summary

A mechanic suffered a crushed fingertip while moving a hydraulic-oil drum by hand dolly across Matterhorn’s platform deck. The dolly slipped while held on an I-beam, trapping his finger against the beam. BSEE identified inattention and inadequate selection of handling aids; evacuation and fingertip amputation followed.
