Summary
BSEE investigates a pipe dropped approximately 40 feet during crane-assisted well abandonment at Eugene Island Block 307A. Two workers avoided the falling pipe; equipment was damaged, with no injuries or pollution. Findings identify inadequate blind-lift controls, communication, hazard analysis and supervision, alongside poorly maintained elevators with a weak latch spring.
