Summary
BSEE investigates a contractor’s injury during boarding shutdown valve testing at Shell’s Auger facility. An unsecured drain hose became pressurised after an equalising valve opened, and its fitting struck his hand and back. Findings identify departure from the approved testing procedure and participation without prior boarding valve testing experience.
