Summary
Investigation of a crane incident during pump back-loading from a platform to a field boat. Inadvertent auxiliary lever operation and failure of the anti-two-block system allowed the auxiliary ball to strike the boom tip, severing its cable. The ball fell into Gulf waters without striking the boat; no injuries or pollution resulted.
