Summary
A crane boom resting on a makeshift support damaged a building roof and fell about three feet, striking the building and damaging its tip. The investigation identified insufficient structural and material strength and inadequate hazard evaluation. Personnel were counselled about planning unusual tasks, including job safety analysis.
