Summary
An investigation describes a floorhand’s facial injury while breaking tubing on Hercules 204. Personnel used pipe wrenches and an unsecured hydraulic tong as a back-up stop; the tong toppled and its cylinder shackle struck him. Findings identify unsuitable tool use, absent job safety analysis, poor planning and failure to stop work.
