Summary
An offshore operator injured his right knee when a stairway step collapsed after oil sampling. The investigation identified failure of its fibreglass support, with ageing and exposure discussed as possible deterioration factors. Repairs included aluminium supports, additional bracing and fastening; other stairwells were inspected.
