Summary
Investigates a drill-pipe stand falling across the derrick during connection work on Discoverer Americas. Misalignment and premature release of pipe-racking arms left the pipe unsecured. Revised procedures required engaged racking arms until connection verification and latched MDDM elevators as a second barrier. No personnel, equipment or environmental harm occurred.
