Summary
Investigation of a float-cell explosion and fire during pipe cutting on a production platform. A torch replaced a dulled cold-cut saw. The report identifies ignition of an air–hydrocarbon mixture, with a probable vapour path near a hatch gasket, and recommends shielding equipment, inerting tank interiors and preventing air infiltration.
