Summary
Investigation of a tubing-handling incident aboard Diamond Ocean Blackhornet in Green Canyon Block 683. Tubing rolled from an uneven pipe bundle after a wooden wedge was displaced, causing crushing injuries and trapping the deck coordinator. Findings address ignored warnings, unsafe positioning, unsecured loads, supervision and failure to exercise stop-work authority.
