Summary
BSEE investigates a floorhand injury during personnel hoisting for a post-jarring top-drive inspection on H&P rig 107 at Ship Shoal Block 349. A snagged secondary safety line pulled against the rising hoist line. Racked drill pipe obstructed the operator’s view, and the required flagger was absent. The floorhand was evacuated by helicopter.
