Summary
Investigation of a floorhand’s serious injuries during tripping operations on Noble Amos Runner. A rotating pipe rack handler trapped and pulled him through its mechanism. Findings address unsigned permits, personnel positioning, poor communication, inadequate supervision and clothing visibility. Contractor recommendations include improved supervision, crew communication, drill-floor check-in procedures and contrasting coveralls.
