Summary
A floorhand fractured an ankle when a personnel basket dropped twice during derrick-cleaning preparations on Ensco 8503. Investigators identified an uncertified utility hoist, improper cable spooling and an unqualified operator. Work instructions and job safety analysis missed hazards; subsequent measures included clearer hoist signs, increased training and supervision.
