Summary
A rigger sustained finger injuries while guiding a crane block during casing cutter recovery at Ship Shoal 224 D. The investigation identified hand guidance instead of push poles, inadequate job safety analyses and unenforced handling procedures. Revised rigging avoided lowering the block through the deck opening.
