Summary
An operator was fatally struck by a rotating bent tubing extension during casing blowdown at approximately 1,400 psi. Gas discharge generated an unscrewing moment, and the assembly subsequently detached. The alert recommends job safety analysis and mandatory procedures for hazardous manual blowdown operations, and cautions against throttling with ball valves.
