Summary
A platform crane boom failed while pulling a workstring during permanent well abandonment. BSEE identified an incorrect load chart, use of static rather than required dynamic limits, worn pin connections and corrosion as contributing factors. No personnel were injured. Recommendations address inspections, load-chart verification and adherence to lifting procedures.
