Summary
An offshore worker inadvertently opened a ball valve while attempting to stop a leak. Released pressure dislodged drain tubing, which struck and fractured the worker’s nose. The alert examines redundant components, inadequate hazard analysis and weak tubing support, recommending consideration of valve selection, management of change and suitable tubing arrangements.
