Summary
BSEE describes three Gulf of Mexico incidents in which stored energy in slings or a trapped shackle caused facial injuries. Two involved short-service employees. Operators and contractors are advised to consider stronger hazard communication, rigging procedures, body positioning, stop-work reinforcement, and additional training and supervision.
