Summary

Two offshore crane accidents during off-loading of a bunkhouse and a rotary table involved underestimated load weights, excessive boom angles and broken weight indicators. Cables parted and loads fell, without personnel injuries. Lessees’ actions include operator training and certification, known load weights, operational indicators, monthly inspections and preventive maintenance.
