Summary
Investigation of a thermowell-flange leak and fire at Mongstad refinery on 3 July 2022. Hydrogen-rich naphtha auto-ignited following release. The report examines flange design, insulation, maintenance omissions, risk analysis and emergency response. Nobody was injured. Material investigations did not establish the underlying cause unambiguously.
