Summary
Investigation of an engine-room fire on Scarabeo 5 while moored on the Njord field. Diesel escaped after a flange bolt fractured and contacted a hot surface; fatigue was considered the most likely fracture cause. Findings address supplier modifications, preventive maintenance, hot-surface protection, Novec activation delays and emergency training deficiencies.
