Summary
Investigation of a cooling-medium leak during insulation removal at Hammerfest LNG. The report examines an unusual left-hand-threaded bleed plug, incomplete valve closure, gas-detector positioning and impairment, and emergency intervention. Leakage lasted about 6.5 hours.
