Summary
This alert describes a mud-pump incident following repairs to an end-cap leak. After pumping restarted, an employee closed a bleed-off valve on instruction. The pop-off valve blew and a downstream pipe tee split, releasing pressure and mud. A nearby employee fell backwards and sustained a hand laceration.
