Summary
A scaffolding foreman fractured his pelvis after a spanner slipped while removing temporary sea fastenings on a platform skid beam. He fell towards a deck 13.5 m below, striking a support beam. The work had changed from scaffold access to harness use without follow-up discussion with permit authorities.
