Summary
A failed hydraulic hose on an Iron Roughneck was investigated by reinstating hydraulics and operating the assembly. A worker approximately 12 feet away was struck by hydraulic oil, which entered his left eye despite safety glasses. Eyewash and subsequent specialist assessment followed; he was found unfit to return to work.
