Summary
Following bull heading and a tour change, an incoming driller opened the annular preventer and attempted to hoist drill pipe while the top pipe rams remained closed. A tool joint struck the rams and the pipe parted at the rig floor. No injuries occurred; the alert identifies poor communication.
