Summary
An offshore rig lifeboat sustained severe stern deck damage during a scheduled weight test using water bags. Stripped threads on a sheave tensioning bolt allowed the sheave to detach, slackening the bow fall wire and dropping the bow into the water. Personnel remained safely onboard the rig under the procedure.
