Summary

A welder’s assistant held centrifuge pump lines and collectors during modifications for an additional mud tank. Instead of a welding shield, he used dark safety glasses and glass from a broken welding mask. The inadequate improvised protection resulted in welder’s flash in both eyes, with pain, photophobia and difficulty opening them.
