Summary
During a rig’s five-year survey, a third-party surveyor tested a lifeboat using an on-load release. When the boat struck the water, a manufacturer-fitted fire extinguisher detached from its bracket and injured the assisting Assistant Barge-Master’s finger. Hospital treatment was followed by restricted work and return to duty.
