Summary
A rig crew preparing for cementing substituted old casing for a missing PVC shaker-bypass pipe. A forklift operator inserted a fork through the casing to lift it from a junk box. The casing caught the box lip, fell off the fork and struck a flagger’s leg, causing a large abrasion.
