Summary
A brass thrust washer fell from a derrick crown sheave, narrowly missing a worker during cementing pipe handling. Inspection found lateral movement of the sheave pin, a leaning sheave and sheared locking plate bolts. Personnel accessed the crown for inspection and repair, and rig operations were suspended.
