Summary
A rig toolpusher was killed while repairing a forklift hydraulic leak beneath a raised load. The load had been raised approximately 10 feet (3 metres). During an attempt to tighten the leaking fitting, it broke off the hydraulic line, causing the load to drop and trap him.
