Summary
Two people were injured during removal of BOP bonnet bolts using a hydraulic torque tool. While operating at its maximum pressure of 10,000 psi, the tool was struck with a sledgehammer to obtain extra force. It failed, releasing stored energy and propelling parts into one person’s hand, arm, face and neck.
