Summary
A strop eye failed while two 400 kg buoyancy blocks were recovered from the seabed, allowing one to rise uncontrolled. Nobody was injured and no damage occurred. The alloy sleeve had corroded after about 11 months underwater. Suggested precautions address prolonged submergence, recovery checks and visual inspection before use.
