Summary
A worker died after trying to restrain a swinging crane-lifted drill-pipe bundle and becoming trapped against a pipe-rack post. The investigation identified missing task analysis, limited offshore experience, absent formal training, inadequate communication with supervision and shortcomings in contractor policies and selection arrangements.
