Summary
A partially inflated 10-tonne parachute lift bag escaped its rigging during surface-supplied diving in approximately 24 metres of seawater. A securing shackle had mistakenly been removed during earlier de-rigging. No injuries occurred. The flash examines inspection shortcomings and single-point securing arrangements, and records model-specific checks, training and work-instruction revisions.
