Summary
A chain hoist failed while crew positioned moonpool frames during mobilisation of multibeam echosounder equipment. The falling frame and carriage damaged stopper bars and brackets without injuries. Investigation identified obstruction by a bracket stopper and chain overstress. Corrective actions included dye penetration testing and revised procedures and job safety analysis.
