Summary
A bolt-tensioning jack nipple was expelled under hydraulic pressure and struck a diver connecting a 24-inch pipeline to a manifold flange, without injury. Investigation identified an incorrectly fitted whip nipple. The flash recommends removing and visually checking used jack and whip nipples, replacing incorrect fittings and then testing at maximum system pressure.
