Summary
A worker died after falling from approximately 18 metres through a gap in a suspended modular scaffold. Plywood intended to bridge the gap was found on the floor. Contributory factors included missing scaffolding training, a supervisor’s assumption of safety, poor communication between subcontractors and an unrecognised walkway barrier.
