Summary
An internal explosion during assembly of oxy-acetylene cutting equipment injured a dive tender, though not seriously. Oil or grease contamination probably caused the explosion. The review identified an improvised manifold, missing flashback arrestors, removed regulators and inadequate cylinder segregation.
