Summary
During diving operations, extending an A-frame without paying out an inactive tugger wire caused its clump weight to enter the snatch block and the wire to part. The weight landed approximately 10 m from divers, without injury or damage. Corrective measures addressed risk assessment, supervision, diver exclusion zones and communication.
