Summary
Two people were fatally injured when a lifeboat fell during recovery after mandatory launch testing on the Auger platform. Support points released and separated; investigation findings were pending. Members may wish to suspend manned launch testing and maintenance or training inside lifeboats unless comprehensively secured by launch prevention devices.
