Summary
A subcontracted employee tripped over a discharge hose at an offshore platform boat landing and fell between the landing and a workboat. He was recovered partially submerged and sustained a fractured wrist. The flash identifies inadequate hose management and unimplemented risk controls, recommending access-route review and consideration of additional trip-hazard controls.

Summary
